There is no good reason for the Sky cancellation form to broken?

After 6 months of poor service from Sky I decided to cancel my service with them and move to Virgin Media. Due to a lack of reception I have twice had engineers out to the house and neither has managed to fix the issue, I can excuse the first chap but I specifically told the call centre in Bangalore that they need to send someone out to reposition the dish only for the young lad to tell me he didn't have the right equipment to do such a job.

After not receiving a follow up call from either visit I decided Sky had taken enough of my hard earned cash for such a poor service and went online to cancel.

Simple enough? You would have thought so, forms are not the most complex items on the Internet right? Well after spending a good half an hour filling out the Sky cancellation form with a the lengthy reason for me dropping their service I received the following message "Sorry, but the form you requested is unavailable, please try again later" (see graphic below). There Is No Good Reason For The Sky Cancellation Form To Be Broken? The annoying thing was when I clicked back on the browser the majority of the fields had been wiped.

You can make your own mind up on this but it is an easy thing to monitor and fix.
